Audio Visual Form

Intended For: Literature Instructors that woud like to request Audio Visual Services for a class or lecture. Before filling out this form please familiarize yourself with the ACMS services already available in classrooms across campus.

Guidelines

1. You are responsible for equipment between start and stop time. Do not leave equipment unsupervised.

2. You are responsible for a $10.00 late fee for each order change or cancellation for AV equipment made less than 48 business hours prior to requested date and time.

3. All orders must be made 2 days prior to the requested date and time or there will be a $10.00 late fee. This is a media services policy.

4. This is a brand new form. Please follow up with the Undergraduate Office at least two working days before you need the equipment to make sure that your order has been placed.
